Key Responsibilities

• Organize and manage resident admissions, discharges, and care transitions
• Support residents in achieving optimal well-being
• Coordinate with interdisciplinary teams, families, and external agencies
• Maintain documentation and regulatory compliance
• Ensure accuracy in insurance verifications and recordkeeping

Qualifications

• Bachelor’s degree in Social Work or related human services field
• 1–2 years of related healthcare experience
• Two years in a similar or long-term care social work position preferred
