About the job

This position provides neurobehavioral health services to elderly residents in long-term care facilities, with a multidisciplinary approach. The therapist collaborates closely with care teams and families to deliver specialized support for neurocognitive and psychiatric conditions.

Key Responsibilities

• Conduct evaluations and provide condition-specific or insight-based therapy
• Coordinate care plans with facility staff, primary care providers, and families
• Act as part of an interdisciplinary treatment team
• Complete documentation in electronic medical records (EMR)
• Engage with families as needed for continuity of care

Qualifications

• Master's degree in Mental Health Counseling or Social Work
• Active state licensure as an LCSW or LMHC
• Strong time management and autonomous working capabilities
• Experience in geriatrics and/or inpatient or extended care preferred
• Familiarity with bedside assessment screens and EMR systems
