Job Summary

• Provide direct nursing care to residents
• Ensure individualized care plans are followed to meet residents’ physical, emotional, and social needs

Key Responsibilities

• Deliver patient-centered care in long-term and rehabilitation settings
• Assess, monitor, and document residents’ health and progress
• Administer medications and treatments according to protocols
• Collaborate on individualized care plans
• Support and educate residents and families
• Maintain accurate documentation and regulatory compliance
• Create a therapeutic and safe environment
• Supervise nursing assistants

Qualifications

• Valid RN license
• Clinical skills in geriatric and rehab care
• BLS/CPR (and ideally ACLS) certified
• Compassionate, team-oriented
