This position is set at a leading long-term care and rehabilitation facility dedicated to delivering compassionate, high-quality care. The successful candidate will become part of a team focused on resident recovery and improved quality of life through individualized care plans.
Key Responsibilities
- Deliver patient-centered care in a long-term rehab setting
- Assess, monitor, and document resident health
- Administer medications and treatments in compliance with facility protocols
- Coordinate with physicians, therapists, and other team members
- Educate and support residents and families
- Maintain documentation meeting regulatory standards
- Mentor nursing assistants and support a safe environment
Qualifications
- Registered Nurse (RN) license
- Geriatric and rehabilitation care experience preferred
- Excellent teamwork and communication skills
- Compassion and attention to patient needs
- Current BLS/CPR certification (ACLS a plus)
