Key Responsibilities
  • Deliver patient-centered care in a rehab and long-term care setting
  • Monitor and document residents' health and progress
  • Administer medications and treatments as prescribed
  • Develop and implement individualized care plans with the team
  • Educate residents and their families
  • Supervise and mentor nursing assistants
  • Maintain compliance with documentation and facility protocols
Qualifications
  • Valid RN license
  • Clinical experience in geriatric and rehab settings preferred
  • Strong communication and collaboration skills
  • Compassionate and team-oriented
  • BLS/CPR required; ACLS a plus
