About the job

Act as a Regional Case Manager focused on coordination and completion of resident assessments for several facilities. Lead Medicare- and MDS-related activities to ensure compliance and high-quality care.

Key Responsibilities
  • Coordinate regional resident assessment processes
  • Ensure timely, accurate MDS and Medicare assessments
  • Conduct facility visits, audits, and provide staff training
  • Monitor assessment schedules and compliance
  • Review and implement policies and procedures
  • Conduct chart reviews and provide consultation on Medicare-related topics
  • Gather information for appeals and support implementation of regulatory changes
Qualifications
  • Registered Nurse with active state licensure
  • Min. two years' experience in long-term care, with MDS and Medicare experience
  • Excellent organizational and computer skills
  • Strong oral communication and leadership
  • Willingness to travel extensively in the region
