About the job
Act as a Regional Case Manager focused on coordination and completion of resident assessments for several facilities. Lead Medicare- and MDS-related activities to ensure compliance and high-quality care.
Key Responsibilities
- Coordinate regional resident assessment processes
- Ensure timely, accurate MDS and Medicare assessments
- Conduct facility visits, audits, and provide staff training
- Monitor assessment schedules and compliance
- Review and implement policies and procedures
- Conduct chart reviews and provide consultation on Medicare-related topics
- Gather information for appeals and support implementation of regulatory changes
Qualifications
- Registered Nurse with active state licensure
- Min. two years' experience in long-term care, with MDS and Medicare experience
- Excellent organizational and computer skills
- Strong oral communication and leadership
- Willingness to travel extensively in the region
