Join our leadership team as a Regional Case Manager/MDS Consultant (RN) overseeing clinical excellence across multiple skilled nursing facilities in Central Alabama. This regional position is responsible for managing MDS and Medicare assessment processes, ensuring compliance, and supporting resident care outcomes.

Key Responsibilities
  • Lead and coordinate MDS and Medicare assessments in assigned facilities
  • Ensure timely, accurate completion of assessments and compliance with regulations
  • Conduct audits of documentation, therapy minutes, and care delivery
  • Support case management for high-risk Medicare Part A residents
  • Train teams on Medicare coverage and documentation best practices
  • Partner with therapy providers for optimized rehab outcomes
  • Participate in appeals, denials, and regulatory initiatives
  • Present recommendations to facility leadership
  • Stay current with regulatory updates and educate teams
Qualifications
  • RN license in good standing (required)
  • Minimum 2 years' long-term care experience
  • At least 1 year in MDS/Medicare roles
  • Strong documentation and leadership skills
  • Proficient with MDS software
  • Willingness to travel regionally
