Legacy Health and Rehabilitation offers an engaging opportunity for skilled nurses to lead and impact resident care as an MDS Coordinator (RN). This full-time, Monday through Friday position focuses on clinical integrity, regulatory excellence, and ensuring accurate and timely interdisciplinary MDS assessments and care plans in compliance with CMS RAI Manual regulations.

Key Responsibilities

• Coordinate, complete, and submit timely and accurate MDS assessments and care plans
• Conduct care plan meetings with residents and families
• Oversee MDS scheduling and maintain tracking systems
• Lead PPS and Medicare meetings, set assessment reference dates
• Ensure compliance with all federal and state regulations related to the RAI process
• Perform data entry, code MDS, and submit required documentation electronically
• Support quality assurance and performance improvement activities
• Coordinate with billing for monthly close-out and assessment completion

Qualifications

• Associate’s or Bachelor’s degree in Nursing
• Current RN license in good standing
• At least one year of experience with Medicare/Medicaid reimbursement and case management
• Familiarity with RAI process and related regulations
• Minimum one year MDS experience
• Strong organizational and documentation skills
