This Licensed Practical Nurse (LPN) role is integral to a long-term care and rehabilitation facility focusing on compassionate, high-quality patient care. The LPN collaborates closely with RNs, physicians, and the broader healthcare team to deliver comprehensive resident care in a supportive and patient-centered environment.

Key Responsibilities

• Administer medications and treatments as prescribed
• Monitor and document residents’ health status and vital signs
• Report changes in residents’ conditions to the supervising RN or physician
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding
• Support rehabilitation plans developed by therapists
• Maintain accurate documentation following facility and regulatory standards
• Provide emotional support and education to residents and their families
• Ensure a safe, clean environment, following infection control protocols

Qualifications

• Current and valid LPN license
• Strong clinical skills, including medication administration and wound care
• Excellent communication and teamwork abilities
• Compassionate and patient-focused approach
• BLS/CPR certification required
