Join a team dedicated to providing compassionate long-term care and rehabilitation services during the night shift. The Licensed Practical Nurse (LPN) plays a vital role in supporting residents’ physical, emotional, and social needs, collaborating with RNs, CNAs, and providers for smooth transitions and consistent, high-quality care.
Key Responsibilities
- Administer medications and treatments as prescribed
- Monitor and document residents’ health status and vital signs
- Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
- Collaborate with the rehabilitation team on care plans
- Maintain accurate and timely documentation per facility and regulatory requirements
- Provide emotional support and education to residents and families
- Ensure a safe and clean environment, adhering to infection control protocols
Qualifications
- Current and valid LPN license
- Strong clinical skills, including medication administration and wound care
- Excellent communication and interpersonal skills
- Ability to work in a team-oriented environment
- Compassionate, patient-focused approach
- BLS/CPR certification required
