About the job
Step into a collaborative team as a Licensed Practical Nurse (LPN) on the evening shift at a rehabilitation and long-term care facility. Provide skilled nursing support, ensuring residents' needs are met with professionalism and empathy during the evening hours.
Key Responsibilities
- Administer medications and prescribed treatments
- Monitor and document residents’ health status and vital signs
- Assist with ADLs
- Support rehabilitation and care plans

Qualifications
- Valid LPN license
- Proficiency in medication administration and wound care
- Effective communication skills and team orientation
- Compassionate, patient-first approach
- BLS/CPR certification required
