About the job

Join a dedicated team at Oak Knoll Health and Rehabilitation, serving residents in a long-term care and rehabilitation setting. As a Licensed Practical Nurse (LPN) on the evening shift, you will support physical, emotional, and social needs, ensuring quality care and a comforting environment for residents.

Key Responsibilities
  • Administer prescribed medications and treatments
  • Monitor resident health status and report changes to supervising RN or physician
  • Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding
  • Collaborate with the rehabilitation team on therapy plans
  • Maintain accurate documentation compliant with policies and regulations
  • Provide emotional support and education to residents and families
  • Ensure safety and infection control in the working environment
Qualifications
  • Current and valid LPN license
  • Strong clinical skills, especially medication administration and wound care
  • Effective communication and interpersonal abilities
  • Team-oriented mindset
  • Compassionate, patient-focused approach
  • BLS/CPR certification required
