Key Responsibilities
  • Administer medications and treatments as prescribed
  • Monitor and document residents’ health status
  • Assist with daily living activities including bathing and feeding
  • Support rehabilitation goals in collaboration with therapy teams
  • Maintain accurate and timely documentation
  • Provide emotional support and education to residents and families
  • Ensure a safe, clean environment adhering to protocols
Qualifications
  • Current and valid LPN license
  • Strong clinical skills in medication administration and wound care
  • Excellent communication and team collaboration

  • BLS/CPR certification required
