Key Responsibilities
- Administer medications and treatments prescribed by physicians
- Monitor and document residents’ health status and report changes
- Assist residents in daily living activities (ADLs)
- Collaborate with the rehabilitation team to optimize recovery goals
- Ensure all documentation complies with facility and regulatory standards
- Provide emotional support and education to residents and families
- Maintain a safe and clean environment, following infection control protocols
Qualifications
- Valid LPN license
- Clinical skills in medication administration and wound care
- Strong communication and interpersonal abilities
- Effective team player
- Patient-centered, compassionate care
- BLS/CPR certification required
