Key Responsibilities
  • Deliver hands-on patient-centered care in a rehabilitation setting
  • Administer medications and treatments per physician orders
  • Monitor and record resident conditions and vital signs
  • Assist with activities of daily living (ADLs)
  • Collaborate with other healthcare professionals and families
  • Maintain accurate documentation in electronic medical records
  • Provide education and emotional support to residents and families
  • Promote a safe and therapeutic environment
Qualifications
  • Valid Licensed Practical Nurse (LPN) license
  • BLS/CPR certification
  • Experience in long-term or rehabilitation care preferred
  • Effective communication and teamwork skills
