Play a crucial role as an Admissions Nurse (RN) by welcoming new residents and coordinating their entry into our facility. Set a high standard for clinical care in a supportive, team-based environment.

Key Responsibilities

• Conduct initial clinical assessments for admissions
• Develop individualized care plans
• Coordinate with families, hospitals, and providers
• Complete required admissions documentation accurately
• Educate residents and families on services
• Collaborate with interdisciplinary teams
• Provide support and guidance during transition
• Engage in quality improvement initiatives for admissions

Qualifications

• Current Registered Nurse (RN) license
• 1–2 years nursing experience preferred
• Knowledge of long-term care regulations
• Strong assessment and documentation skills
• Excellent communication skills
• Compassionate, resident-centered approach
